What you will need
the VšZP mobile app (Health Wallet MAXI),
a voucher from your eye doctor (prescription proof),
payment receipt from the optical store,
completed preventive health check-ups.
Four-step process
Eye doctor. Visit a VšZP-contracted ophthalmologist – the examination must be within 90 days before purchasing glasses.
Optical store. Pick up your glasses at a VšZP-contracted optical store – for example, IMOOPTIK. The payment receipt must cover the full purchase, not just a deposit.
Application. Submit your application within 60 days of payment using the VšZP mobile app in the Health Wallet MAXI. Upload the payment receipt and your doctor's voucher.
Preventive check-ups. After submitting your application, verify in the app that all required check-ups are completed.
Required preventive check-ups
General practitioner (adults),
Pediatrician (children),
Gynecologist (women),
Urologist (men).
If a check-up is missing, don't worry – you can still submit your application and complete the check-up later. Deadlines depend on the application date:
Applications submitted between Jan 1 and June 30 → check-ups must be completed by Sept 30 of the same year,
Applications submitted between July 1 and Dec 31 → check-ups must be completed by March 31 of the following year.
